Your forms, done before you arrive

Filling out your intake forms before you arrive means we can spend your first visit on your history, your movement and your questions instead of a clipboard.

Call or email us and we will send your forms.

There is no download on this page. To get your intake paperwork ahead of time, call (312) 489-8579 or email info@movementguild.com and we will send it over so you can fill it out at home before your first visit.

If you would rather do it here, come in early and we will hand you a clipboard. Either way the point is the same: your appointment time should go to your history and your movement, not to forms.

How the first visit actually starts.

Request Your Forms

Call or email and we will send your intake paperwork. Completing it at home is the single biggest thing you can do to protect your own appointment time.

Arrive A Little Early

Give yourself a few extra minutes to park, find the door and get checked in. If you still have paperwork to finish, come earlier than that.

Check In At The Desk

Our patient care coordinator takes your ID and insurance card, confirms your details and lets your therapist know you are here. Then your evaluation starts on time.

Wear something you can actually move in.

Your therapist needs to see the area that hurts and watch how the rest of you moves around it. Street clothes make that harder than it has to be.

Lower Body Issue

Wear or bring shorts. Hips, knees and ankles are far easier to assess and treat when the leg is visible.

Shoulder Or Neck

A tank top or sports bra lets your therapist watch how the shoulder blade, ribs and neck move together.

Low Back Or Pelvis

Loose athletic wear works best. Skip jeans, belts and anything with a stiff waistband sitting across the area.

Bring Your Shoes

Athletic shoes matter if walking, running, lifting or squatting is part of what brought you in.

Ask us for the paperwork.

Leave your details below and mention that you need intake forms. We will email them to you and confirm your appointment time in the same reply.

Faster still: call (312) 489-8579 or email info@movementguild.com and ask for the intake packet.
